Reduce Social Isolation – Social Prescribing

Connecting with others in your community improves your mental, physical, emotional and social health. The social prescribing programme helps you to feel connected while working on your social, mental and physical health.

Who is Social Prescribing for?

The programme is for anyone over 18 who might feel isolated, stressed, anxious or experiencing low mood. Using resources within the community, we help you to meet people, find support or take part in new activities.

We can help you connect with people through different groups and activities, such as:

  • Exercise and yoga classes
  • Reading and art groups
  • Gardening
  • Volunteer opportunities
  • Relaxation groups
  • Computer classes

How does Social Prescribing work?

  • Step 1: You or your GP, community worker or health professional refer you to our service.
  • Step 2: We listen to the barriers you might face and what you are interested in
  • Step 3: We work together to find local activities you enjoy and are right for you
  • Step 4: We organise taking part in different activities with you
  • Step 5: We check in with you to see how things are going

We also have a specific Social Prescribing service for Older People who have been involved with their local ICPOP (Integrated Care Team) which is aligned to St James’ Hospital. This service is known as the Community Connector for Older Persons.
